1-check five rights
right patient
right name of medication
right dose
right route
right time
2-hand washing before giving medication
3-never touch medication with your hands
4-wear gloves
5-donnot use drug with abnormal colour or odor or consistency
6-check expiry date of medication
7-never record any medication before giving it
8-record if the patient refuse any medication and report to physician
9-check the patient history of allergy to any medication
10-for antibiotics
-peform allergy test before giving it
-(cefotax )should be given with caution to patients with renal impairment
-the drug must be administered separately .avoid mixing with other drug
-monitor the patient for any side effects
-observe for any abnormal response to medication
11-for antiacid
-drug should be given before meal
-obseobserve for liver function because it may lead to increase in liver enzymes
-obseve the patient response to medication
-observe for any sideeffects
-(zantac)is excreted via kidney so it must be taken with caution in patient with renal impairment
12-for insulin(antihyperglycemic)
-perform blood glucose test before giving it
-change site of injection regularly
-instruct the patient to eat after giving it
-check random blood sugar regularly
13-diuretics
- intake and output chart
-check blood pressure before and after giving it
-assess for any side effect such as nausea,gastric discomfort,skin rash,or
orthostatic hypotension
-(lasix)is contraindicated in patient with anuria and in patient with histoy of hypersensitivity to furosemide
avoid mixing drug with other drugs
-check electrolyte level after giving it
